The short answer

  • A hospital management system (HMS) runs patients, doctors, staff and money in one place: registration, OPD queue, medical records (EMR), prescriptions, billing and reports.
  • A clinic needs 5 modules on day one. Pharmacy, IPD, OT and insurance can wait until you really run them.
  • One-year licences listed on IndiaMART in October 2026 ranged from ₹29,999 to ₹3,50,000. Cloud systems charge monthly instead. Ask for every line of the quote: messages, training, moving your data, the price in year two.
  • Before you pay, run one real OPD morning on the trial and time three tasks: a new walk-in, a repeat prescription, a bill.

What a hospital management system does: one patient's visit

The easiest way to understand an HMS is to follow one patient through it. Every step below is a module. If a step happens on paper in your hospital today, that is a module you need.

  1. 10:40
    Registration

    Mr. Joshi gets token 14

Reception finds his old file by phone number, so no second file is made. Walk-ins and booked patients join one queue.

  2. 10:48
    OPD queue

    Vitals at the counter

BP 168/98 is flagged before he reaches the doctor. The doctor's screen shows he has waited 8 minutes.

  3. 11:02
    EMR

    The doctor sees his story

Penicillin allergy, two current medicines from another doctor in the hospital, and last month's sugar report. No paper file to search for.

  4. 11:09
    e-Prescription

    A checked prescription

Generic names, his allergy checked, advice printed in Gujarati, and a PDF on WhatsApp.

  5. 11:12
    Billing

    One bill

Consultation (GST-exempt) and medicines from the pharmacy (taxed) on the same bill, paid by UPI.

  6. Day 3
    Lab + follow-up

    The report comes back

The HbA1c is high, so it is flagged for the doctor, and his follow-up reminder goes out on time.

HMS modules: what you need now and what can wait

Buying every module on day one is the most common way to overpay. Start with what your team does every day, and add the rest when you really run that service.

What does a hospital management system cost in India?

₹29,999 to ₹3,50,000

One-year licences for hospital management software listed on IndiaMART in October 2026, from a desktop-only system to a multi-speciality package.

Prices are hard to compare because vendors charge in three different ways:

  • Cloud subscription: a monthly or yearly fee, per doctor or per clinic. Nothing to install, updates included.
  • Licence: a one-time or yearly licence for software installed on your own computers, often with a yearly maintenance fee.
  • Custom build: a system made only for you. The most expensive, and you depend on one developer for every change.

The licence or subscription is only part of the cost. Ask every vendor to put these lines in writing: setup and training, moving your old records, SMS and WhatsApp messages, extra users or locations, hardware such as a token display screen (listed from about ₹10,000 each), and the price in year two.

12 questions to ask before you buy

Ask each vendor the same questions, and write down the answers. The last line of each card is the answer that should worry you.

  1. 1

Walk-ins and appointments in one queue

Most Indian OPDs mix booked patients with walk-ins. A system built only for appointments pushes reception back to a paper list.

AskCan reception add a walk-in in under 30 seconds?

Red flag"Walk-ins are entered as appointments."

  2. 2

    One record across doctors

A patient seen by the physician and then the surgeon should have one history, with every current medicine visible to both.

AskIf two doctors prescribe on the same day, does the second one see the first prescription?

Red flagEach doctor keeps a separate patient list.

  3. 3

Each role sees its own work

Reception should not read clinical notes, and every change should show who made it.

AskCan I give reception access without showing notes? Is there a change history?

    Red flagEveryone shares one login.

  4. 4

    Prescriptions that meet NMC rules

The medical council's ethics regulations ask doctors to write generic names, legibly and preferably in capitals, and to show their registration number on prescriptions, certificates and receipts.

AskDoes the print show generic names and my registration number? Can advice print in Hindi or my state's language?

Red flagOnly brand names, only in English.

  5. 5

    GST on the right lines

Health care services by a clinical establishment are exempt from GST. Medicines from your pharmacy and services such as cosmetic procedures are taxed, and both can be on one bill.

AskCan one bill hold exempt and taxable lines? Is there a GST report for my accountant?

Red flag"Put GST on everything, or on nothing."

  6. 6

Patient data under the DPDP Act

Under the Digital Personal Data Protection Act, 2023, your hospital is responsible for patient data (the Data Fiduciary). The DPDP Rules, 2025 phase in duties such as reporting a breach, with a detailed report within 72 hours.

AskWhere is the data stored? Is it encrypted? Who at your company can see it?

Red flagNo clear answer on where the data is.

  7. 7

    ABDM, if you need it

The Ayushman Bharat Digital Mission lets patients link records to their ABHA health ID. It is voluntary for private clinics, but some government schemes and states ask for it.

AskIs the system ABDM-integrated, and which ABDM milestones has it completed?

Red flag"Coming soon" with no date, when your scheme needs it now.

  8. 8

    Standard medical codes

The Health Ministry's EHR Standards (2016) recommend codes such as ICD-10 for diagnoses and LOINC for lab tests. Coded records are easier to report and to move.

AskAre diagnoses coded, or only typed as free text?

Red flagDiagnoses exist only as free text.

  9. 9

    Your data in, and out

Moving in should not mean typing old patients again. Leaving should not mean losing them.

AskWill you import our patients from our current software or Excel? Can we export everything, any time?

Red flagExport only on request, or for a fee.

  10. 10

Works on what you already have

Browser-based systems run on the laptops, tablets and phones you own. Installed systems may need a server in the hospital and someone to look after it.

AskDoes it work on a phone? What happens when the internet drops for ten minutes?

Red flagNeeds a new server and a yearly visit to update.

  11. 11

The full price, year one and year two

Compare the price for your real size: your doctors, your staff, your locations, your messages.

AskWhat will we pay in year one and year two? Is there a setup fee, a lock-in or a yearly increase?

Red flagThe price for year two is "to be decided".

  12. 12

    Support your reception can reach

Most questions come from reception on a busy morning, not from the owner.

AskHow do we reach support, in which languages, and how fast do you reply?

Red flagSupport only by email, only on weekdays.

Test it with one real OPD morning

A demo shows the vendor's best path. A trial shows yours. Before you pay, run one real morning with your own reception and time three tasks. Time the same tasks on your current system or paper, so you have a number to compare.

TaskGood after one day of practiceYour time

Repeat prescription with 2 current medicinesUnder 2 minutes____

Which is the best hospital management system in India?

There is no single best system, and lists that rank one are usually written by a vendor. The best system for you is the one that passes the 12 questions for your size and wins the timed morning above. Shortlist three, run the same morning on each, and decide on the numbers.

Doccine is one of the systems you can test. It is the operating system for healthcare in India: a hospital management system (HMS), EMR and clinic management system in one. It runs the OPD of clinics and hospitals today, and inpatient care (IPD) is coming next in the same patient record. It works in the browser on any laptop, tablet or phone, stores data in India, prints GST bills and prescriptions in English, Hindi or Gujarati, and our team moves your old records free of cost. Put it through the same 12 questions.

Questions people ask

What are the modules of a hospital management system?

The core modules are registration and OPD queue, medical records (EMR), e-prescriptions, billing with GST, and lab reports. Hospitals add staff roles, pharmacy and stock, IPD (admission to discharge), OT scheduling, insurance claims and HR.

How much does hospital management software cost in India?

It depends on the model. One-year licences listed on IndiaMART in October 2026 ranged from ₹29,999 to ₹3,50,000; cloud systems charge a monthly or yearly fee per doctor or per clinic. Add setup, training, data moving, messages and hardware to compare fairly.

What is the difference between an HMS and an EMR?

An EMR is the patient's clinical record: history, allergies, prescriptions, vitals and reports. A hospital management system includes the EMR and also runs registration, the OPD queue, staff access, billing and management reports.

Is ABDM integration mandatory for a private clinic?

Taking part in the Ayushman Bharat Digital Mission is voluntary for private clinics today. Some government schemes and states ask for it, so check what applies to you before you choose.

Cloud or installed: which is better for a small hospital?

Cloud systems need no server, update themselves and work from any device, but need internet. Installed systems work offline inside the building but need a server, backups and someone to maintain them. Ask each vendor what happens when the internet or the server goes down.
